Left hand in a downward palm block, step forward with right foot and perform straight spearfinger thrust. Twist to release from a grab spinning round 270 degrees anti-clockwise.

I envisage it as being applied in this manner:

The Opponent does a double lapel grab.

Strike down with the left hand raking in towards your chest and striking Lung 5 et al on the way in.

Knee with the right leg and slam the right hand foward, grabbing their windpipe/throat. Grip hard. This is where it gets nasty.

Twist the grabbing hand 90 degrees anti-clockwise, this brings the hand into a knifehand like postion with the edge of the hand facing up towards their chin. Push forward with all your might into their throat trying to drive the knife hand into their adams apple.
